Any visible difference that cannot be accounted for by simple accent items (such as shield hung above the shop in Haggerfel but not Fort Veldon) is considered to be a different model.  This includes "mirror-image" buildings such as the ones at Outland Wharf, since they are visibly different (even though mirror-imaged) they are considered two models.

Any building which appears to be a part of the town is considered to be a part of the town, such as the guard towers at Caille and Basar.  Basically if it is too far away to be included in a screenshot of the town then it is not part of the town.
